Formativelabz Inc provides this website to explain its courses and answer questions.

Interest and enquiry forms

The website links to a Google course-interest form and has a partnership enquiry form. Interest is not enrolment: no payment, reserved place or learner access is provided by an interest form.

Google course-interest form

The linked Google form collects your name and email, optional mobile/WhatsApp number, course of interest, country/city and an optional note. Formativelabz Inc uses the responses only to reply about your enquiry. Google hosts the form and stores the responses for Formativelabz; Google's privacy terms also apply. This is not consent for marketing emails.

Partnership enquiry form

The partnership form collects partnership type, organization name, location, business email, contact person, optional phone and website, and your objective. Formativelabz Inc uses these details only to reply. The records are stored privately on our server, not in public website files. This form is for organizations; do not enter information about children.

Free session emails

The "Tell me about upcoming free sessions" form at formativelabz.com/free-sessions.html is for people aged 18 or over. It collects your first name, your email address and, if you choose, your country. It also records that you ticked the consent box, the exact consent wording you saw, the date and time, the page, and your IP address, so we can show that you agreed. The box is not ticked for you, and we only email people who tick it.

Formativelabz Inc uses these details only to email you about Formativelabz sessions and courses. You can unsubscribe at any time using the link in each email, or by writing to reach@formativelabz.com. We do not sell this list. Records are stored privately on our server, not in public website files. We ask for no phone number, student ID or school name. Signing up is not enrolment.

The AI Payoff Check

The AI Payoff Check at formativelabz.com/payoff-check/ is a separate tool with its own sign-in. It collects: your company name, industry and province or country; your answers, the hours and hourly figure you enter, and an optional logo; an optional quote or log you upload; your email address and a password, which we store scrambled and cannot read; and answers from staff you invite, which are anonymous and shown only once at least 3 people have replied. We use this only to prepare and check your report. A Formativelabz person reads your answers and any file you upload before the report is released. No AI model reads your answers or files. The records are stored privately on a server in Canada (Amazon Web Services, Montreal region). Uploaded files are deleted 30 days after your report is approved. We delete the rest if you ask. The tool sets one sign-in cookie after you sign in and keeps your unsaved draft in your own browser. It has no advertising and no analytics. We do not sell this information. Your report is general information from your answers, not legal advice.

Formativelabz Inc is based in Calgary, Alberta. We handle personal information under Alberta's Personal Information Protection Act (PIPA). Where information crosses a provincial or national border, Canada's Personal Information Protection and Electronic Documents Act (PIPEDA) can also apply. If you are outside Canada, the law where you are may also apply to you. Questions about privacy go to reach@formativelabz.com. Please do not enter or upload patient or other health records, or personal details about customers or staff that the report does not need. The tool does not ask for them. If you are a custodian under Alberta's Health Information Act, do not put health information into it. This page describes what we do. It is not a promise that any law has been met for your business.

Access, correction and deletion

Email reach@formativelabz.com to ask about information you have sent us, or to request correction or deletion. We will explain any information we need to keep. We do not sell enquiry records or add them to a marketing mailing list.

Emailing us

Our contact link opens your email service. If you email us, we receive your email address and whatever you include, and use it to respond. Do not send passwords, payment details, health records or a child's identifying information.

Younger learners

AI Future Scholar is intended for Grades 9-12. An adult or guardian should enquire using their own contact details, not a child's identifying information. Enrolment and guardian consent remain separate.

Technical information

Optional Google Analytics measures page visits and enquiry-link clicks after you choose Accept. It uses analytics cookies and shares technical visit data with Google. Advertising features are disabled. No form-field values are sent. Decline keeps the site working. Use Analytics choices to withdraw consent or change your choice. Google may separately process technical information when you open its form. Our hosting provider keeps standard access logs.
